Prior to starting your semester at Walden, please use this page as a handy resource for contacting the right Walden team member who can best answer your questions and facilitate your requests. In addition to a useful directory of department contacts, please find helpful links for submitting a Household Information Form, Student Health Form, and resources for staying connected with Walden.
FACTS FAMILY PORTAL (Required)
All families and students (both new and returning) will need to log into the new FACTS Family portal to update their demographic, medical, and family information in preparation for the 2023-24 school year.
Walden Family information forms (Required)
To login into your FACTS Family Portal, please use these instructions to create an account. For parents, make sure you create an account using the email that Walden has on file for you. For students, make sure you use your Walden School email address.
WALDEN FAMILY INFORMATION FORMS (REQUIRED)
Once logged in, all families (both new and returning) will need to update their family and student information using the built-in web forms. To access those, click on “Web Forms” under the School section in the menu on the left side of the FACTS homepage. Then select Walden Family Information Forms. There you will find links to student and parent information forms, the student medical form, technology agreement forms, transportation permission, and emergency contact forms. Please review each of those forms and edit any information you find that is incorrect.
Sign up to receive Walden email communication
It is HIGHLY RECOMMENDED for both parents/guardians to join our mailing list to receive the Walden newsletter and vital email communication. Families will not only receive the weekly newsletter, but important campus updates, event and activity information, Division Director highlights, head of school communication, and more.
IMPORTANT: CHECK YOUR PROMOTIONAL OR SPAM FOLDERS
Although we strive for perfection, sometimes Walden emails arrive in your promotional or spam folders. Please check these folders periodically throughout the week to ensure you do not miss out on any important communication.

IMMUNIZATIONS (Required)
Updated Kentucky Approved immunization certificates are required to be submitted to our Healthcare Coordinator, Leslie Logsdon.
Due to Kentucky regulations, updated certificates must be on file in order for your child(ren) to attend Walden School.
Medication Drop-off and Paperwork (Required)
Any student that will be taking prescription medication during school hours must contact our Healthcare Coordinator, Leslie Logsdon, at llogsdon@walden-school.org to set up a day and time to come in and arrange for medication drop-off and paperwork submission. Paperwork and medication will not be accepted on the first day of school.
Technology Agreements and Policies
REQUIRED FOR NEW AND CURRENT FAMILIES: In preparation for the 1:1 device learning program, please submit technology agreement forms for each student through the FACTS Family Portal. Families also have the option of enrolling in our Device Protection Plan ($40 per/device). Click the image links below to get started.

Walden Lunch Program
COST: Lunch is provided in the cost of tuition; no extra charges will apply.

K-8 STEAMworks After School Program
Under the leadership of Walden School Librarian and Director of Enrichment Programs, Ms. Christen Brown, the STEAMworks After School Program will be in full swing with a variety of fun and engaging activities.

Walden Family Tree
We'd love for you to share your stories and photos!
